MMCH offers immunizations BY APPOINTMENT ONLY for both children and adults at the Wellness Clinic located in downtown Batesville.

Tuesdays: 12:30-7 p.m.
Thursdays: 10 a.m. – Noon and 1-5 p.m.

Directions to the Wellness Clinic
From I-74, go south on Highway 229 (also called Walnut Street) towards downtown Batesville. At the first stop sign, turn right onto Boehringer then immediately turn left into our parking lot.

Child Immunizations
For children 17 years of age and younger, consent must be provided by a parent or legal guardian. We understand that a parent may not always be available to be with the child. In this circumstance, you may complete a Consent to Immunize a Minor Authorization form. Forms may be faxed to (812) 932-3374. Please bring your child’s personal immunization record with you or immunizations may not be able to be given.

Adult Immunizations and Tuberculin Skin Testing
An adult is anyone 19 years of age or older. Vaccines may be limited in supply, so you may want to call in advance to verify your vaccine is in stock. Below is a list of the available vaccines and Tuberculin skin tests:

• Hepatitis B
• Human Papillomavirus (HPV)
• Measles, Mumps, Rubella (MMR)
• Meningococcal
• Pneumonia
• Shingles
• Tetanus/Diphtheria (Td)
• Tetanus/Diphtheria/Pertussis (Tdap)
• Tuberculin Skin Test
• Typhoid (Oral)
• Varivax (Chicken Pox)
